Lifestyle Continuity Program Up to two 30-min visits with the doctor/month (in office or telehealth), email, and texting. Access to Validic for fitness-data tracking. There is a $150 first visit comprehensive evaluation and registration fee. Discounts available for patients of other DPC practices.	$48.00 per month
A La Carte Initial Consultation for Lifestyle Medicine For those wishing to pay per visit rather than subscription. Initial consultation includes a comprehensive evaluation of your current lifestyle habits and environment, biometric data collection (including blood work), and the prescription of a detailed plan to help you reach your goals.	$175.00 one-time fee
A La Carte Follow-Up Visits for Lifestyle Medicine For those wishing to pay per visit rather than subscription. Visits are 30-min and can be conducted in the office or via telehealth. Does not include blood work or expanded access (email/text).	$75.00 one-time fee
Super Market Visit This includes review of your 1-week meal plan and grocery list as well as guided grocery shopping to help with produce selection and reading labels.	$200 ONE-TIME FEE
