Medication Refill Requests

Click the button below to request a refill for one or more of your medications.  WE REQUIRE A ONE WEEK NOTICE FOR ALL MEDICATION REFILLS.YOU MUST COMPLETE ALL BLANK FIELDS IN THE REFILL REQUEST FORM.  If you are submitting a form for a refill that is needed sooner than one week from today, or if you do not fill out all of the necessary fields, delays are likely to occur.

Patients of any Direct Care Physicians of Pittsburgh practice are automatically granted membership to Blueberry Pharmacy, which is a cost-plus pharmacy that dispenses generic medications at wholesale costs with free delivery or shipping once a quarter.  The pharmacists at Blueberry get to know their patients very well and are also available as consultants who can assist with (1) learning to use your medication - especially helpful for inhalers or injectibles, (2) finding the best pricing for higher-priced brand name medications, and (3) reviewing your medication list to make suggestions based on health or cost. Click here to learn more.

OFFICE VISIT: 30 mins

This is NOT a telehealth visit.

  • Follow-up 1-2 chronic conditions
  • Simple acute issue (cold, flu, UTI, rash, etc)
  • PLEASE DO NOT SCHEDULE A PAP SMEAR OR PROCEDURE HERE; USE THE 1 HOUR APPT TYPE BELOW.

OFFICE VISIT: 1 hour

This is NOT a telehealth visit.

  • Follow up more than 3 chronic issues
  • 2 or more new issues
  • Procedure (PAP smear, boil, etc)

OFFICE VISIT: Complete Physical (1 hour)

This is NOT a telehealth visit.

Annual meeting with your member to make sure you are up-to-date on all screening tests, bloodwork, and vaccines.  A full physical exam will be performed.  If you have a form that needs to be completed, please bring it with you to the appointment.

OFFICE VISIT: Pre-Surgery Consultation (1 hour)

This is NOT a telehealth visit.

Preoperative consultations are required for some surgical procedures.  Usually, you will receive paperwork from the surgeon outlining the necessary steps.  If bloodwork, chest xray, and/or EKG are required, the surgeon should have given you scripts for these.  If you are uncertain, please refer to your paperwork or call the surgeon's office.

The preoperative office visit and any necessary testing typically must be done within 30 days of the procedure.

OFFICE VISIT: Vaccination

THIS LINK IS NOT FOR SCHEDULING A COVID-19 VACCINE. 

Brief appointment for the administration of vaccine(s). We carry the following vaccines: flu (ages 3 and up; preservative-free), tetanus/pertussis (whooping cough), meningitis A & B, pneumonia (both Pneumovax and Prevnar13), HPV (Gardasil), and shingles (Shingrix).  We now offer a full range of pediatric vaccines for newborns through teens.

TELEHEALTH VISIT (aka E-VISIT)

These are 30 minute appointments designed to deal with minor problems (such as allergies, colds, backache, pink eye, rashes, urinary symptoms, yeast infections) OR follow-up issues previously discussed with your doctor at a face-to-face visit. Instructions will be included in the scheduling confirmation email.

PHONE CALL

We allot 30 minutes for phone appointments.  These are useful for discussing acute mild symptoms, following up a chronic issue, or discussing test results.
